Gross Description -- A Testicular Mass


GROSS DESCRIPTION:

The specimen consists of an ovoid, cystic mass weighing 1350 grams and measuring 28.0 x 18.0 x 10.5 cm with 5.0 cm of attached nodular spermatic cord, ranging 1.0 to 2.5 cm in diameter. The external surface of the tumor is tan-pink and focally hemorrhagic, with a tense glistening capsule. The cut surfaces of the tumor are markedly heterogeneous, ranging from firm, white-tan, slightly bulging tissue to soft yellow areas (Images 01, 03). There are large areas of necrosis and hemorrhage with 100 cc of dark viscous fluid (Image 02). There are multifocal cystic areas, including a large cyst measuring 5.0 x 4.5 x 2.5 cm with a bulging inner wall. There are several thick fibrous bands running throughout the tumor. No normal testicular tissue is grossly identifiable. The tumor appears to be encapsulated but extends into the tunica albuginea. The epididymis, spermatic cord and surrounding soft tissue are dissected and reveal gross involvement by tumor. The spermatic cord surgical resection margin is grossly free of tumor.
