Gross Description -- HIV Positive Man Died Following Bleeding from Mouth and Nose


GROSS DESCRIPTION:


EXTERNAL EXAMINATION:
The body is extremely cachectic. There is dried blood around the nose, mouth and right ear. The skin shows multiple purpural hemorrhages and petechiae covering the chest, upper abdomen, shoulders, face, neck, upper and lower extremities and back.

HEPATOBILIARY SYSTEM:
The liver capsule is slightly roughened. The hepatic parenchyma is firm, light tan with tan-white, thin bands of apparent fibrous tissue.

DIGESTIVE SYSTEM:
The gastric mucosa has flattened rugae and multiple petechial hemorrhages. The stomach contains approximately 300 cc of coffee ground material. The intestinal mucosa is dusky dark red and contains approximately 2-3 L of bright red blood in the small intestine and dark black thick material in the colon. The pancreas shows hemorrhage and extensive fat necrosis extending on to the mesentery of the small intestine.

RETICULOENDOTHELIAL SYSTEM:
The spleen weighs 610 grams. The splenic capsule is smooth, grey and is adherent to the diaphragm and portion of the small bowel mesentery. The parenchyma is red with vague white nodules up to 0.3 cm. The thoracic lymph nodes are firm and uniformly grey-black. The abdominal lymph nodes are soft and pink-tan, up to 1.0 cm.

Cardiovascular system and genitourinary system are grossly unremarkable.
