Microscopic Description -- Lymphoplasmacytic Lnfiltrate

IMMUNOHISTOCHEMISTRY:

Antigen/Antibody Usual Reactivity Result
CD20/L26 B cell Moderate number of positive cells in association with plasma cell nodules.
CD3 T cells Moderately numerous positive cells.
Anti-lambda B cell Subset Numerous positive cells within plasma cell nodules, and scattered plasma cells outside of nodules.
Anti-kappa B cell Subset Moderate number of positive cells outside plasma cell nodules, very few within them.
EBV-ISH(EBER) Epstein-Barr virus Weak stain; a moderate number of cells appear positive..

MOLECULAR STUDIES:

In order to further characterize both the clonality of the plasma cells, and the Epstein-Barr virus association with the lesion, genotypic studies were performed. PCR analysis for Immunoglobulin heavy chain rearrangement showed a monclonal band (Image05), further supporting the clonality of the plasma cells present. Epstein Barr virus terminal repeat analysis showed clonal Epstein Barr virus DNA in the lesion (Image06). This supports the presence of a clonal EBV infection.
