Final Diagnosis -- Cirrhosis (Hepatitis C)


FINAL DIAGNOSIS:

PART 1: NATIVE LIVER (1500 GRAMS), ORTHOTOPIC LIVER
TRANSPLANTATION -
A. ADVANCED CIRRHOSIS WITH MIXED MACRONODULAR AND
MICRONODULAR PATTERNS AND MODERATE CHOLESTASIS.
B. MILD MACROVESICULAR STEATOSIS AND EVIDENCE OF
STEATOHEPATITIS.
C. INFARCTED MACROREGENERATIVE NODULE (deeper levels
and reticulin stain to follow).
D. S/P CHOLECYSTECTOMY.
PART 2: ALLOGRAFT LIVER, POST-PERFUSION NEEDLE BIOPSY -
A. CHANGES OF MILD PRESERVATION/ISCHEMIC INJURY.
B. NO CONFLUENT NECROSIS SEEN.
Comment:

The cirrhosis is at an advanced stage and the histologic features are consistent with the clinical history of chronic Hepatitis C infection. Additional morphologic changes raise the possibility of concurrent steatohepatitis; this should be evaluated clinically as an etiologic factor.

ADDENDUM #3 (1 Reticulin) The reticulin stain demonstrates focal absence of the reticulin fibers. Again, no evidence of malignancy is seen.
