Microscopic Description -- Diarrhea and Urinary Frequency


MICROSCOPIC FINDINGS:

The tissue examination of the small intestinal implants showed necrotizing granulomatous inflammation (Figures 8 and 9)

The acid-fast stains on the small intestinal implants showed acid-fast positive bacilli (Figure 10 and 11).

The Grocott stains were negative for fungi.

MICROBIOLOGICAL FINDINGS:

Stool smear for ova and parasites was negative.

Stool for MAI was negative.

Stool smear for acid-fast bacilli was negative.

Urine culture grew Streptococcus viridans, Yeast and Diphtheroids and a Gram stain of this urine showed multiple gram negative and gram positive morphotypes and confirmed the feculent nature of the urine. Few weeks later Acid-fast bacilli were found on this urine culture. DNA probe was positive for Mycobacterium Tuberculosis Complex and negative for Mycobacterium Avium Complex. Microorganism was sensitive to Streptomycin, Rifampin, Ethambutol, Pyrazinamide, and resistant to Isoniazid.

Stool cultures showed no WBC's present on Methylene Blue smear. No normal gram negative flora was present. No Salmonella, Shigella, Campylobacter or E.coli 0157:H7 were identified. But, few weeks' later Acid-fast bacilli were found on culture. DNA probe was positive for Mycobacterium Tuberculosis Complex and negative for Mycobacterium Avium Complex.
