After submitting the application, please mail these items to the specific fellowship program at one of the addresses provided below.
  1. Updated curriculum vitae including current address, current and future training programs
  2. Official USMLE or COMLEX transcript (a PDF copy is acceptable)
  3. Three or more letters of recommendation, one of which must be from your residency program director
  4. Medical School Transcript (a PDF copy is acceptable)
  5. ECFMG certificate, if an IMG

The application, supporting documents, letters of recommendation, and interview must be completed by December 31 of the year prior to the training year that the candidate is applying for.

Completed applications will be independently reviewed by each fellowship program. After review, candidates will either be declined or offered an interview. Interviews will include meeting with 2 fellowship directors from outside of the program. Interview reports are sent to the central fellowship office where they will be collated and the results sent to the individual fellowship director. If individual fellowship director decides to offer the candidate acceptance and funding is available an offer letter and contract will be issued.
