“Improving Patient Safety by Examining Pathology Errors”

This session will include both dissemination and discussion of findings from the currently active AHRQ funded research project entitled “Improving Patient Safety by Examining Pathology Errors”. This project is a multi-institutional demonstration project involving the voluntary sharing of de-identified anatomic pathology quality assurance (QA) data related to anatomic pathology diagnostic errors for the purposes of performing formal root cause analysis for errors and testing the usefulness of laboratory interventions designed to decrease errors. Specific QA metrics that will be discussed will include cytologic-histologic correlation rates, numbers and types of amended reports, and frozen section-permanent section discrepancy rates. Findings will be presented by the overall project Principal Investigator (Stephen S. Raab, MD) as well as by pathologist investigators from other participating laboratories.
