How to Use Requisition Form
The Requisition Form can NOT be sent to our Laboratory via the internet. It has to be printed, filled out with all required information, then mailed with the sample to the address listed on Test Specifications page.
There are two separate requisitions for hematopathology testing. One requisition is to be used for bone marrow, peripheral blood and body fluid specimens. The second requisition is to be used for lymph node and solid tissue specimens.
