Microscopic Description -- Eight Year Old Boy with Skin Rash and Proteinuria



MICROSCOPIC DESCRIPTION

About 40 glomeruli are present in the three cores of renal tissue. All the glomeruli are involved and show diffuse mesangial cell and matrix proliferation. Focal fibrinoid necrosis of the tuft and neutrophilic infiltration are noted in rare glomeruli. Some glomeruli show hypercellularity, lobulation, and thickening of the basement membrane (BM) with rare areas of BM splitting. Fifty to sixty percent of the glomeruli show fibrosis and epithelial crescents in various stages of development. These crescents are most easily seen on the PAS stain.Occasional glomeruli show fibrous obliteration. Interstitial fibrosis, infiltration of lymphocytes, plasma cells, and tubular atrophy are noted focally.
