Gross Description -- Liver Mass


GROSS DESCRIPTION:

The specimen is is received unfixed, labeled "liver resection, right lobe", and consists of 3,530 gram, 30.0 x 20.0 x 12.0 cm right lobectomy (segments 5, 6, 7 and 8). The capsular surface is mottled brown-tan, smooth and glistening with focal fine fibrinous adhesions. The tumor can be seen expanding the capsular surface over much of the lateral aspect of this specimen. Nowhere does it grossly breach the capsule. Multiple subcapsular nodules are present ranging up to 12.5 cm in greatest dimension. One of these nodules can be seen at the margin of resection near the medial aspect of the gallbladder fissure. A frozen section was performed here. Cut surface reveals a 26.0 x 18.0 x 10.0 cm multilobulated white firm mass with focal soft yellow necrosis and hemorrhage. The edges of the tumor are grossly circumscribed, and the tumor bulges from the cut surface. The uninvolved liver is non-cirrhotic. The hepatic venous margin, hepatic arterial margin, portal venous margin, and bile duct margins are grossly free of tumor. Gross photographs were taken. Digital images are captured, portions of the specimen were sent to the Pittsburgh Cancer Institute, a portion sent for DNA analysis, a portion sent for cytogenetics, and tissue is frozen in bulk and in OCT, and a portion is submitted for electron microscopy.
