Case 170 -- Non-Hodgkin's Lymphoma and Renal Failure

PATIENT HISTORY:

The patient is a 61-year-old man with advanced, systemic non-Hodgkin's lymphoma, presenting with renal failure and microscopic hematuria of several weeks duration. Pertinent history is remarkable for chronic NSAID use. Review of systems is remarkable for edema.

Kidneys are bilaterally enlarged sonographically. The clinical differential is lymphomatous infiltration of kidneys versus interstitial nephritis secondary to NSAID use.

Urine Sediment Hyaline, rare fine granular cast
5-10 RBCs/ HPF
10-20 WBCs/HPF
Urine Culture Negative
Urine Protein 114 mg/24 hrs
