MICROSCOPIC DESCRIPTION:
Paraffin-embedded brain sections stained with hematoxylin and eosin showed granulomatous meningitis with many giant cells. A well-circumscribed zone of acute/subacute necrosis was identified in the corpus callosum, consistent with a vascular territory infarct. Many chronic inflammatory cells and giant cells were seen surrounding branches of the anterior cerebral artery over the area of infarction. Although a specific obstructive or an occluding vascular abnormality was not identified, dense perivascular infiltrates in the narrow sulcal spaces may have impaired local flow.
